The system you will lean on at seventy-five, not the clinic you see at sixty.
Healthcare is the dimension where the cheap end of a retirement list quietly stops being an option. Ranked on the system you can reach as a resident foreigner, the order inverts almost completely against a cost ranking: the places that treat you best are, with one or two exceptions, the places that cost the most to live in. That trade is the single most consequential one on this site, so the monthly cost sits in every row here rather than on another page.

What the rating measures, and what it deliberately ignores
This rates the care a foreign resident can actually obtain — hospital quality in the cities where retirees settle, whether a resident can join the public system at all or must hold private cover, and whether serious cases stay in-country or get flown out. It is not a ranking of national health systems in the abstract. A country can run an excellent public system that a foreign retiree is locked out of for five years, and that lockout is what matters to you.
It does not price your policy. Private cover is quoted against your age and your history, and the gap between a premium at sixty and the same cover at seventy-two is larger than the gap between most countries on this list. Several destinations here also cap new enrolment at an age you may already have passed. Those are per-person facts and they sit behind the member layer on each country page, because a brochure rate is worse than no rate.
One structural warning that no rating captures: joining a public system usually requires legal residency, and legal residency usually requires the visa. If the visa route is fragile, the healthcare position is fragile too, however good the hospitals are.
